Supervising Family Case Manager

Reports to: Director of Program Operations

JOB SUMMARY: Supervise family case management staff who provide services to African and Caribbean families and collaterals living with HIV/AIDS. Coordinate response to psychosocial, medical, mental health, family planning, permanency planning, legal, housing, and entitlement requirements of HIV+ individuals and affected family members. Conduct comprehensive assessments and develop comprehensive service plans for individuals/families infected/affected by HIV/AIDS.  Assist the clients and department with creating permanency planning objectives including but not limited to health care proxies, standby guardianships, and living wills.


  • Perform intakes for individuals who are HIV+ or living with AIDS
  • Conduct comprehensive psychological assessments, identifying current and future needs of all family members in the household of HIV+ individual(s)
  • Develop a comprehensive service plan for family support system, coordinate with identified community resources, including primary and HIV and specific health care, education, entitlements,  legal services, child care, permanency planning and financial planning
  • Provide counseling with permanency planning goals such as health care proxies, standby guardianships, and living wills
  • Advocate for family service needs to ensure appropriate delivery of services
  • Maintain confidentiality of client records
  • Provide translation/interpretation services as necessary
  • Supervise two family case management staff
  • Identify gaps in areas of service needs
  • Develop linkage agreements with new service providers as needed
  • Organize and facilitate focus groups, support groups and develop volunteer peer support
  • Ensure that the program objectives as defined by HIV Care Services & Aids Institute (funders) and that service types are met in the monthly reports
  • Ensure that monthly reimbursable service units are achieved
  • Monitor monthly service targets by tracking and comparing service logs with the data entered in both E-Share  reporting system and the AIDS Institute Reporting System (AIRS).
  • Submit monthly program reports by deadline
  • Supervise social work interns/volunteers



Requires New York State LMSW

  • Minimum two years Post-MSW work experience. SIFI- certified or eligible preferred.
  • Minimum of two year's experience in HIV/AIDS family case management or other commensurate disability-related experience, in supportive family counselling.
  • Must be bilingual in English/French (strongly preferred) or in English/Spanish.  Fluency in one or more major African language preferred.
  • Must demonstrate an understanding of and sensitivity to cultural issues, and social problems of African and Caribbean refugees and immigrants.
  • Excellent communication skills, spoken and written. Basic computer skills a must.  Familiarity with E-Share and AIRS a plus.



Salary range: $52,000 - $54,000, commensurate with experience. Benefits include health, dental, retirement and life insurance plans.
