HIV/TB Coordinated Care

Since 2001, our innovative HIV/TB Housing and Case Management Program has helped individuals coping with dual infection to initiate treatment and obtain sustainable housing to support their health.

The program is staffed by a case manager who coordinates housing and subcontracts HIV/TB medical management to the National TB Center at Harlem Hospital. Services provided as part of this multidisciplinary team patient-management approach, include: TB case finding and contact investigation; housing placement and short-term rental assistance; HIV/TB medical management and directly observed therapy; case management; health education; and referrals for additional supportive services.

Clients enrolled in our HIV/TB program are also served by our legal, medical interpretation, food and nutrition, and support group programs.
